The second section contains the Daily Journal Entry
pages, starting with a one-day example. A month's worth
(30 days) of Daily Entries is provided to record the
day-to-day activities that occur during the patient's
hospital stay. Sometimes a day in the hospital can be
very busy! As shown in the example, each Daily Entry
consists of four pages, two for recording hospital staff
names, comments and medications, and two for recording
tests & procedures, questions & answers and notes.
As the physicians make their rounds each day, write down their name (and add them to the Physicians list later), their medical specialty,
the time of their visit and their comments. Space is also provided for the equally important nursing staff and any medications given that day. On the
next page you can record Tests & Procedures that have been ordered that day. Finally, the page entitled Q & A provides space for any questions you
may have for the physicians and other medical staff. Using the Daily Entries every day in this manner will provide you with a priceless
source of medical information concerning the patient's progress toward recovery.

This section includes pages for tracking the best part
of the patient's hospital stay, going home! Use the
Discharge Planning page to record information
that will be important after the patient has been discharged
(i.e., recommendations and treatments from physicians,
nurses and social workers that will be followed at home).
On the Home Medications pages document all prescription
medications, including the prescription date, purpose,
dosage and dosage frequency. To prevent the possibility
if dangerous drug interactions, carefully document all
prescription medications on these pages and show the
list to the patient's doctors during future office visits.
Nearly all patients will require Follow-up Appointments,
so three pages are included for recording appointment
dates, physician names and comments.
